Job Description & Requirements16849174 RN → Home Health\nYears of Experience:2 Years\nRequired Certifications: RN licensed in WV; BLS\n\nCommon diagnosis / Types of patients/patient demographics: COPD, CHF, DM, mainly adults, rarely peds.\n\nAdditional Job Duties/Utilization Review involved? Admissions, use of Oasis\n\nRT Equipment (vents, Cpap, BiPap, etc): Trilogy, trachs, oxygen, BiPAP occasionally\n\nExperience or 'Must have' skills: Autonomous,\n\nHow many patients do they see per day? We see 5-6 patients daily for 8 hrs employees, depending on distance they travel for the day. 12 hr employees will be 8-9 depending on travel.\n\nManager/Supervisor: Director and Supervisor available 24/7 via phone\n\nHome Health/Hospice Aids? Have aides\n\nShifts & Scheduling (Do you allow self-scheduling? Block scheduling?) Through week- M-F 8a-4:30p. Weekend position Saturday, Sunday, Monday 8a-8p\n\nWeekend Requirements: None for weekend position, may have 1 every 6th weekend for day\n\nHoliday Requirements: On rotational basis\n\nIs there a call or standby requirement? No call\n\nFloating Requirements: No floating,\n\nScrub Color/Dress Code: Royal Blue Scrubs\n\nUnit/Department specific orientation: Will be completed day 1-3
